Eligibility:
Native African American students (US citizen) enrolled in an accredited school of nursing program in any of
the five counties in the Greater Kansas City area
Application must include:
• Completed Data Information form (fill out below - online form)

Late submissions will not be accepted
The below information must be sent via mail
A minimum of $1000 scholarships will be awarded
• 2 Proofs of Identification (i.e. copy of Driver's license, student ID)
• Official school transcript (good academic standing with a minimum 3.0 GPA) Unofficial transcripts will not be accepted.
• A 500 word double spaced typed essay addressing:
- reasons you chose nursing as your professional career
- the impact of nursing in health care (be specific)
- what this award means to you
• Two letters of recommendation letters (personal, professional, or academic)
Recipient(s) must agree to:
- Become an active student member of local chapter
- Attend at least 5 meetings the first year
- Participate in at least one community screening event
- Agree to support student recruitment for the organization
